Sedalia School District 200's Free Physical Night is back after a one-year hiatus. It is scheduled for 6 to 8 p.m. Wednesday, May 26, in the Smith-Cotton High School gym.

Those attending must enter through the front gym doors and check in at the table in the lobby.

Students must arrive with a Pre-Participation Evaluation Form with pages 2-3 completed with the help of their parents. The nurses and doctors will complete page 1.

After the doctors and nurses provide the evaluation, students must leave their form at the main table at center court. Requests for copies should be made in writing to the athletic office.
